Brazilian Butt Lift (BBL) - Buttock Fat Injection & Augmentation
Buttock aesthetics has become a popular global trend in recent years, widely recognized across the world as the "Brazilian Butt Lift" (BBL).
Historically, the naturally voluminous buttocks characteristic of Latin and African genetics gained significant visibility through public figures, turning this look into a highly requested aesthetic procedure among women. Trends regarding buttock size and shape fluctuate over time; while larger, attention-grabbing silhouettes were preferred in past years, today’s ideal leans toward a more natural yet full-contoured appearance.
While standard classification identifies roughly four distinct buttock shapes, I utilize a tailored classification system in my surgical practice to optimize outcomes. You can find more detailed information on this topic in my blog post titled "Buttock Shapes and Types." References like the "Jennifer Lopez butt" or today’s "Kim Kardashian butt" are widely known and frequently used by patients to describe their aesthetic goals. Patients seeking a prominent, full, and eye-catching result often reference these iconic shapes to express their expectations. Looking back at ancient Anatolian civilizations, historical depictions often pair wide, full hips with large, pendulous breasts. While large, sagging breasts are no longer considered desirable today, a prominent, voluminous buttock has evolved into a widely appreciated aesthetic ideal.
Methods Used in Buttock Augmentation
There are two primary surgical techniques used for buttock augmentation:
-
Autologous Fat Injection (BBL / Brazilian Butt Lift): Transferring the patient's own fat harvested from other areas of the body.
-
Buttock Implants (Gluteal Prosthesis): Surgical placement of specially manufactured gluteal implants.
For a detailed comparison on when to choose implants versus fat injection, you can read my dedicated blog post on the subject.
Buttock Fat Injection (BBL)
Fat injection is the most preferred method for gluteal augmentation. Using liposuction techniques under sterile conditions, fat cells are harvested—primarily from the abdomen, waist, and other suitable donor sites—and subsequently injected into the gluteal region.
Roughly 30% to 40% of the injected fat is naturally reabsorbed by the body over time. This retention rate varies between individuals and can even differ across tissue layers within the same area.
To achieve a more lasting and impactful result, my surgical approach focuses on maximum slimming of the waist area. Achieving a narrow waist creates a dramatic waist-to-hip transition, making the buttocks appear naturally fuller and more voluminous. Thus, liposuction plays a pivotal role in overall buttock reshaping. Furthermore, if a patient desires a significantly larger shape, we over-inject fat during the procedure to account for the expected natural reabsorption. Since fat reabsorption during the healing phase may not occur completely symmetrically, mild asymmetrical variations can temporarily arise. If a patient envisions a very large and high-volume transformation, a secondary fat injection procedure may be required.
Buttock Implants (Gluteal Prosthesis)
Gluteal implants serve as an effective alternative for patients who lack adequate body fat for transfer. Specially designed silicone gluteal implants are utilized, typically placed within the submuscular plane.
Pre-Operative Preparation
Depending on the patient's overall health and surgical plan, pre-anesthesia evaluations are scheduled, and pre-operative photographs are taken.
In my practice, I emphasize having a comprehensive, detailed discussion regarding the target shape prior to surgery. Approaching a BBL procedure merely as a choice between "large" or "small" is not realistic. Altering gluteal volume does not simply change body shape; it influences clothing fit, social perception, and overall presence. It is essential to thoroughly discuss these broader visual and lifestyle impacts alongside the physical changes. To gain deeper insight into this process, you may read my article: "Why Understanding What the Patient Truly Wants Is So Important in Buttock Reshaping."
Surgical Process
The duration of the surgery ranges between 3 to 5 hours, depending on the number of donor sites targeted for liposuction. A hospital stay of 1 to 2 nights is typically sufficient. I generally prefer using surgical drains and usually remove them prior to discharge. Wearing a compression garment (corselet) is essential after this procedure. Patients must wear targeted, light-compression garments for at least 4 weeks. Strenuous athletic activities are off-limits during these first 4 weeks, as they carry a risk of inducing bleeding and swelling. Since fat harvesting accounts for a major portion of a BBL, the initial recovery closely mirrors standard liposuction recovery.
Recovery & Post-Operative Care
Using a BBL pillow (buttock support pillow) and avoiding sleeping on your back are key measures to minimize fat loss during early recovery. I recommend strictly following these precautions for at least the first 20 days.
Because the procedure heavily relies on liposuction, overall recovery follows the liposuction timeline. Swelling begins to subside around weeks 3 to 4, with final results settling over a total period of 6 months.
Specialized post-operative massages are recommended during the initial months to support tissue healing and smooth contours.
